At this time, we only serve clients residing in Colorado, Oregon, Maryland, Delaware, Virginia, Vermont, Florida, and Connecticut. After scheduling your appointment, you will receive an email instructing you to complete intake paperwork and consent forms. These forms must be completed at least 24 hours before your appointment to avoid cancellation. There is a $250 fee (full appointment cost) for no-shows for the initial appointment. Please note that if you are approved for our ketamine treatment program, you will be charged the treatment fee shortly after the appointment. The Safe Haven Health Ketamine Treatment Program has a fee of $250 per month, including unlimited appointments with your provider, personalized ketamine prescription and delivery, and support from your provider and support coach. Payment must be made before your medication is delivered.
